You may be wondering what the benefit changes are in 2008 for the Panama Canal Area Benefit Plan. We want to help you! Below are the highlights of the 2008 benefit changes:
  • Your share of the non-Postal premium will increase by 3% for Self only and for Self and Family.
  • We added coverage for newborns and children up to age 10 once every five years for routine hearing screenings, testing, diagnostic evaluations and treatment, including hearing aids.
  • We added the Osteoporosis Disease Management Program to the listed educational classes and programs that are covered to include detection, early diagnosis, and treatment of osteoporosis and osteoporosis related diseases for female members 65 year or older, female members between 60 and 64 with predisposition factors and plan members with chronic low back pain.
  • POS routine medical check-ups covered in full are now two annually rather than one each six months.

What is a Personal Health Profile?

A Personal Health Profile (PHP) is a summary record of patient health information generated by appropriate claims information. Included in this information is a series of patient preventative benefits (immunizations, laboratory and radiology testing) that the patient has had done over the past twelve months.

 

What is the purpose of providing a PHP?

The purpose of a PHP is the support of continuing, efficient and quality integrated health care. This summary of patient specific health events should allow patients to track their compliance with the treatment and preventive programs they have available as benefits and should allow them to take a more proactive attitude towards self-care and informed decision–making activities.

 

How to request your PHP?

Beginning January 1, 2008 members will be able to request their PHP in person or on-line at http://www.axa-assistance.com.pa. Once the member’s information and identity is verified a print out of the PHP will be immediately issued.

  • In Person:
    Member may request the print out of his or her PHP with an appropriate I.D. by visiting the Panama Canal Area Benefit Plan at AXA Assistance, Via España, Edificio Via España 120, Panta Baja, Panama City, Republic of Panama.
  • Online:
    Member may request his or her PHP online at http://www.axa-assistance.com.pa. When member submits a request for their PHP online, an e-mail with PHP request information is automatically generated. Member can then pick up a copy of their PHP in person with an appropriate I.D.

*GENERAL NOTE: IF MEMBER CANNOT RETRIEVE HIS OR HER PHP THEMSELVES, I.E., BED RIDDEN MEMBERS, HE OR SHE WILL NEED TO PROVIDE AN AUTHORIZATION LETTER TO A SECOND OR THIRD PERSON IN ORDER FOR THE PHP PRINT OUT TO BE DISTRIBUTED.
